First: I have to need add an textfield on phone numbers page in my account. I don't know how to do this. where can I add some html code.

second: insert the textfield's value to mysql database.

You need to customize liferay existing jsp by using hook. For updating the database you need to update the EditUserAction.java for getting the value and upddating the database table using service.

The jsp ou need to check is:
portlet/enterprise_admin/common/phone_numbers.jsp

Struts Action:
com.liferay.portlet.enterpriseadmin.action.EditUserAction
